Plasden Court

Plasden Court now offers a 24 hours service for 15 flats to individuals suffering from severe mental illness.

The individual support packages are centered on the Recovery Star.

Prompt consideration is given to those individuals who are judged as currently being inappropriately placed in hospital, residential or nursing home environments.

All referrals to be sent for approval in regards to funding, via the placement panel. Once funding is identified the referrals will then be passed to Brenda McGrath, Scheme Manager, Plasden Court, Praxis Care, for further assessment of the individuals needs/suitability for the scheme.

Plasden Court also operates an outreach service to four individuals who suffer from severe mental illness. Support comprises a similar service to that offered within Plasden Court but on an outreach basis. All referrals should also follow the process above.
