Each person we engage with will have a tailored care package to support their return to live within local communities in a Supported Living setting, with staff being involved with developing each Service User’s confidence and abilities to live as independently as possible. Services are close to each other with support from a local office base and we are growing across the region. Our expertise is in working with people with complex and multiple needs, there will be challenges on the way as outcomes are worked towards but we will be there for each person’s journey. Praxis Care wants to make a real difference in people’s lives by creating opportunities and collaborating with other agencies to promote the best possible outcomes for everyone we engage with.
